The package from post#38 includes it's own prog_ufs_firehose_8998_ddr.elf firehose file. I tried flashing both V30_UNBRICK.ZIP and US99820a.kdz extracted files from post#38. Not sure if I could have try anything else in linux because running qdl returned device not detected. I've followed the instructions in the OP in both windows and linux. I am also assuming it has been bootloader unlocked and converted to a us998 because AT&T is not a carrier option where I am but the firmware state is an educated guess at best. I know it's a H931 because of the branding on the case. I received the phone in this state and I don't have any information about how it became this way. I assume a reboot caused it to disconnect. Pressing power, vol-, vol+ in any combination or all together results in the phone temporarily disconnecting and reconnecting in 9008 mode. As soon as it is connected to the pc, it's detected in 9008 mode. I have a H931 V30 stuck in Qualcomm H-USB QDLoader 9008 Mode. After more than a week of research and testing, I still not been able to unbrick my phone.
